The Los Angeles Aging Disability Resource Connection (ADRC) No Wrong Door System service is your single point of entry into the long-term services and supports system. We proudly serve older adults, people with disabilities, caregivers, veterans, and families.

Our core purpose is achieved by building strong community partnerships, providing services through a dedicated person-centered approach, and actively reducing barriers to accessing necessary care.

Aging and Disability Resource Connection (ADRC) services are provided locally by contracted agencies and serve older adults, adults with disabilities, family caregivers, and residents in long-term care facilities throughout the State

Core Services

ADRC is here to help navigate long-term support options. Four core services are offered, providing personalized assistance designed to help you live independently in the community:

We assist in identifying your goals and needs through person-centered counseling, coordinating access to public and private long-term services and supports.

Receive comprehensive resource information, follow-ups, and warm hand-offs to key agencies and supports.

Expedited access to services for individuals at risk of institutionalization, generally for 90 days or less.

Support for people currently in a hospital or nursing facility wishing to transition back and receive care at home.
