Aging and Disability Resource Connection (ADRC) | Community Investment Department

The Central & South LA Aging Disability Resource Connection (ADRC) No Wrong Door Service serves as a single point of entry into the long-term services and supports system for older adults, people with disabilities, caregivers, veterans, and families. This purpose is achieved by building community partnerships, providing services using a person-centered approach, and reducing the number of barriers to accessing services.

The ADRC also offers a comprehensive Referral and Resource Directory through the City of LA's 211 LA health, human and social services hub.

The ADRC is focused on what is known as Long-Term Care Needs. Long-term care can be many things: housing, benefits, In-Home Support Services, Community Living, Enhanced Information and Referral, Hospice, Home Modification, and much more.

Some of the services they offer include:

Transition Services
helping persons with disabilities and persons 60+ move from restrictive to less-restrictive environments
Options Counseling
assist in identifying goals and needs through person-centered counseling and coordinating access to public and private-funded long-term services and supports in the community
Information, Referral and Assistance Services
comprehensive resource information, follow-ups, and referrals via “warm hand-offs”
Service Coordination
in crisis or urgent situations, expedited short-term coordination access to services and support for individuals at risk of institutionalization, generally for 90 days or less
